上苦利增劇，太子晝夜不離側，或累日不食，發有變白者。上泣曰：「汝能孝愛如此，吾死何恨！」丁卯，疾篤，召長孫無忌入含風殿。上臥，引手捫無忌頤，無忌哭，悲不自勝；上竟不得有所言，因令無忌出。己巳，復召無忌及褚遂良入臥內，謂之曰：「朕今悉以後事付公輩。太子仁孝，公輩所知，善輔導之！」謂太子曰：「無忌、遂良在，汝勿憂天下！」又謂遂良曰：「無忌盡忠於我，我有天下，多其力也。我死，勿令讒人間之。」仍令遂良草遺詔。有頃，上崩。
